• In order to receive a Senior cord, you MUST do the following:

• -You MUST QUALIFY for a Senior cord
• -Cord Form MUST be SIGNED by the CLUB SPONSOR BEFORE you PURCHASE
• -Purchase cords through RevTrak at $12.00 ea. (unless stated “Paid from Dues”)
• -RevTrak receipt MUST be attached to the cord application
•               NO REFUNDS! Please make your purchase CAREFULLY

• Completed Cord order forms are due to the front office by Monday, January 26, 2026
