Senior Cord Application

 

In order to receive a Senior cord, you MUST do the following:

  1. You MUST QUALIFY for a Senior cord
  2. Cord Form MUST be SIGNED by the CLUB SPONSOR BEFORE you PURCHASE
  3. Purchase cords through RevTrak at $12.00 ea. (unless stated “Paid from Dues”)
  4. RevTrak receipt MUST be attached to the cord application
  5. NO REFUNDS! Please make your purchase CAREFULLY
  6. Completed Cord order forms are due to the front office by Monday, January 26, 2026
